New Parent Questionnaire

This questionnaire is prepared to help us support your child as she/he enters Wasatch Academy. At Wasatch Academy, we desire to form a cohesive and supportive environment. Some of the questions in this questionnaire are difficult. This questionnaire is a key to communication between you and Wasatch Academy. Therefore, it is important for you to be as candid as possible. This questionnaire will be shared with your child's Advisor, Dorm Parent, and Head of School. This will not become a part of your child's permanent record at Wasatch Academy.
